Referral Form

If you know someone who could benefit from biblical counseling, you’re in the right place. This referral form is designed for pastors, ministry leaders, and Christian business owners who want to connect individuals or couples with trusted, gospel-centered care.

Referring someone does not guarantee an appointment, but it begins the process. Once you fill out this form an email will be sent to the person you are referring with instructions on how to create an account on our portal. When they sign in they will arrive at their dashboard where they can fill out the necessary forms to get started. We will contact them from there. If everything checks out they will be given access to the calendar where they can schedule their first appointment.
